Welcome Home Care Services · What We Provide

A client visiting with a family member in a living room

A safe place to stay, and a team working on what comes next

A stay here covers the practical side of recovering after a hospital discharge. Facility staff are on site around the clock, and a case manager works with clients on appointments, benefits, resources, and planning for the next placement. This is a non-medical setting, so clients manage their own medications, hygiene, and daily activities while they are with us.

  • 24/7 Facility Support

Facility staff are available around the clock to provide oversight, respond to day-to-day needs, and help maintain a safe and supportive environment. This is non-medical support. Clients in our program manage their own medications, hygiene, and any medical equipment without hands-on assistance from staff.

    24/7 Facility Support
  • Case Management

We help clients address practical needs throughout their stay, including appointments, benefits, identification, community resources, and discharge planning. When housing support is needed, we connect clients with housing resources and coordinate with referral and community partners to support a safe next placement. Planning begins early, and clients are expected to participate throughout the process.

    Case Management
  • Care Coordination

We coordinate with referring hospitals, health plans, case managers, and outside healthcare providers to support continuity of care after hospital discharge. Our team assists with follow-up planning, communicates with the organizations involved in the client's care, and connects clients with mental health, substance use, and other community resources when needed. All medical and behavioral healthcare is provided by outside providers. Our role is coordination and connection, not treatment.

    Care Coordination
  • Daily Meals & Basic Needs

Clients receive daily meals and access to basic household and hygiene necessities during their stay. Meals are shared in a common dining area. Nobody has to work out where the next meal or a bar of soap is coming from while they are getting better.

  • Transportation Coordination

We help coordinate transportation to follow-up healthcare appointments and other necessary services during the client's stay. By identifying transportation needs early and connecting clients with available transportation resources, we help reduce barriers to attending appointments and continuing care after hospital discharge.

    Transportation Coordination
  • Benefits Assistance

We provide clients with information and guidance on benefits and assistance programs they may qualify for, including Medi-Cal, SSI, and SSDI. Clients remain responsible for completing their applications and providing the required documentation, while our team helps connect them with the appropriate agencies and resources.
