A client settled into daily life during his stay

A safe place to heal after a hospital stay

Welcome Home Care Services takes in people who are well enough to leave the hospital but have nowhere safe to finish getting better, and we care for them until they do.

Facility staff on site around the clock. Contact us for referrals, program eligibility, and current bed availability.

Who we are

Some people leave the hospital still needing rest and support staying on track with their medications and follow-up care, but with no safe place to get any of it. Recuperative care is the room in between. We take those people in.

Welcome Home Care Services provides short-term recuperative care for adults who are medically stable and appropriate for a non-medical setting but need a safe place and additional support after leaving a hospital or other healthcare setting.

Our six-bed facility in Westchester gives clients a comfortable place to stay while they rest, receive daily meals, and work with our team on appointments, benefits, resources, and plans for what comes next.

We work with hospitals, social workers, discharge planners, case managers, health plans, healthcare providers, and community partners to help make the transition out of the hospital easier.

What we provide

Case management, care coordination, and a safe place to stay. This is a non-medical setting, so clients manage their own medications, hygiene, and daily activities while they are here.

Referrals and admissions

Written for discharge planners working against a clock, and for the partners who send us their hardest placements.

  1. Send the referral

Email our team at admin@whcsla.com and send the available referral and discharge information, including the information needed to understand the client's current needs.

  2. We review the referral

Our team reviews the client's needs, program eligibility, appropriateness for our setting, and current bed availability. If additional information is needed, we will follow up with the referring organization.

  3. We coordinate admission

If the client is accepted, we coordinate admission directly with the hospital, case manager, social worker, or other referring organization.

Submission of a referral does not guarantee admission. Referrals are reviewed for program appropriateness, required authorization or funding, and current bed availability.
