Time-sensitive discharges
If the discharge is happening today or tomorrow, email the referral to admin@whcsla.com rather than using the message form below. Include the discharge date so our team can answer on current bed availability and on whether our setting can safely support the client.
For hospitals and referring partners
A patient may be ready to leave the hospital and still not have a safe place to continue getting better.
Welcome Home Care Services gives medically stable clients a short-term place to stay while our team helps coordinate appointments, services, resources, and the next steps after discharge.
We work directly with hospital social workers, discharge planners, case managers, and other referral partners throughout the transition.

Read who may be appropriate for our programWhen you send a referral, please include:
- Completed referral form
- Available hospital or healthcare discharge documentation
- Current medication list and discharge instructions
- Relevant information needed to understand the client's current needs and determine program appropriateness
- Insurance, authorization, or funding information when applicable
- Referring contact information and anticipated discharge date
